We are seeking your input on ways to make Grace a more accommodating and accepting church home for children, teens and adults facing physical, mental, behavioral or emotional challenges, as well as their teachers, family, and caregivers.  There are two versions of this survey; click the appropriate link to access the survey:

1. Intended for completion by families of and caregivers for those with special needs, as well as for other interested members. 
2. Intended for completion by Grace teachers and leaders.

NOTE:    While you may complete this survey anonymously, it will be more helpful if you identify yourself and those in need.  Please know that Rev. Carol will not share information on specific individuals or families with anyone except those Grace leaders and teachers who have a need to know this information in order to provide the appropriate accommodations.  Those involved in this initiative will be informed of the requirement for discretion and respect of privacy.  If you prefer, you may contact Rev.
